We will discuss a case of a 50 year old female patient with End Stage Renal Disease and enteric hyperoxalosis after bowel resection, where the initial echocardiogram suggested a restrictive cardiomyopathy. The case depicts the complimentary value Cardiovascular Magnetic Resonance, genetic testing and PYP scanning in a case with restrictive physiology which was confirmed to be cardiac oxalosis by endomyocardial biopsy. The differential diagnosis of findings by Cardiovascular magnetic resonance and PYP scanning in the setting of a restrictive cardiomyopathy with the impact on management decisions will be discussed. Additionally the value of genetic testing in cardiomyopathies will be discussed.
